VIAL is a physician-owned medical practice offering cosmetic injectable treatments, including neuromodulators and dermal fillers. Our services are medical in nature and are provided under the direction of a licensed physician.

Every new client begins with a consultation. Treatment is offered only where a provider judges it clinically appropriate, and we may decline to treat.

You must be at least 18 years old to book an appointment or use this website. By booking, you confirm that you meet this requirement and that the information you provide is accurate and complete.

Appointments are booked through our online scheduling system. To hold an appointment, a valid payment method is required at the time of booking. No deposit is taken and no charge is made at the time of booking. Payment is taken at checkout after your appointment.

Appointment availability varies week to week. Business hours listed on this website are office hours for calls, texts and booking, and do not indicate provider treatment availability.

Cancel or reschedule with more than 24 hours notice to avoid a cancellation fee.

Appointments cancelled with less than 24 hours notice, and appointments where you do not attend without notice, may incur a fee charged to the payment method held on file. The current fee is disclosed to you at the time of booking.

We will make reasonable efforts to notify you of any change to your appointment.

Payment is due at the time of service unless otherwise agreed in writing. We accept the payment methods displayed at checkout. Prices are subject to change without notice, and any price quoted at consultation applies to that treatment plan only.

You agree to disclose accurate and complete information about your medical history, medications, allergies and prior treatments. Withholding relevant information may make treatment unsafe.

Follow all pre-treatment and aftercare instructions given to you. Contact the practice promptly if you experience an unexpected reaction.

Injectable treatments are medical procedures with variable outcomes. Individual results depend on anatomy, product, dosage, technique and factors outside anyone’s control. Nothing on this website, said at consultation, or shown in before and after photography constitutes a guarantee, warranty or promise of any particular result.

Nothing in these terms limits or excludes liability that cannot be limited or excluded by law, including liability for professional negligence in the provision of medical care.

These terms are governed by the laws of the State of Indiana, without regard to conflict of law principles. Any dispute arising under them is subject to the exclusive jurisdiction of the state and federal courts located in Indiana.

We may update these terms from time to time. The “Last updated” date at the top of this page will be revised accordingly. Continued use of this website or our services after a change constitutes acceptance of the revised terms.
